Keyhole Top Surgery for Very Small Chests

A small incision below the areola, minimal scarring

Keyhole Top Surgery for Very Small Chests

Keyhole top surgery removes breast tissue through a small incision along the lower edge of the areola, often combined with liposuction. It leaves the least visible scars but is only suitable for a small number of people with a very small chest and elastic skin.

What is keyhole top surgery?

Keyhole top surgery is the mastectomy technique with the smallest scars: breast tissue is removed, often together with liposuction, through a short incision below the areola, and no skin is cut away. It is only suitable for very small chests with very elastic skin and nipples that are already well positioned.

The name refers to the small "keyhole" opening: the surgeon makes a short semicircular incision along the lower border of the areola and removes breast tissue through it. The procedure is very often combined with liposuction, which removes fatty tissue and helps shape the chest contour.

Unlike other techniques, keyhole surgery removes no skin, and the areola is usually not significantly reduced or repositioned. The nipple stays attached to the underlying tissue, which improves the chance of keeping sensation, although this cannot be guaranteed. Because no skin is removed, the result depends almost entirely on how well the skin retracts.

Top surgery for small chests: who is a keyhole candidate?

Of all top surgery techniques, keyhole has the narrowest eligibility criteria. It is usually only offered when all of the following apply:

CriterionTypical requirement
Chest sizeVery small (roughly A cup or smaller)
Excess skinMinimal or none
Skin elasticityVery good
Nipple positionNo sagging; nipples already well positioned
Areola sizeAlready small or acceptable as it is

If even one of these criteria is not met, the surgeon will rather consider the periareolar technique, which removes a ring of skin and reduces the areola, or double incision. See our guide on choosing a technique for a detailed comparison. Even with a small chest, eligibility must be confirmed by a surgeon after assessment. How keyhole mastectomy works

  • General anaesthesia, with a typical duration of around 2 hours, sometimes shorter or longer depending on liposuction.
  • Tissue removal through the incision below the areola, with the contour refined by liposuction.
  • Stay: often one night at the clinic, sometimes same-day discharge depending on the surgeon's protocol.
  • Drains: not always needed; if used, usually for a few days.
  • Compression vest for several weeks – especially important so the skin adheres to the chest wall.

Because the result depends so much on the skin, many surgeons assess it carefully before agreeing to keyhole: they check how far the skin stretches, whether there are stretch marks, and how the chest looks when you lean forward. If you have been binding for many years, mention it, as this can influence skin quality.

Scars and results

With keyhole top surgery the scar is limited to the lower half of the areola's edge and, once mature, is often hard to see. Keyhole leaves the most discreet scars of all techniques. The cosmetic result, however, depends heavily on skin quality: if the skin does not retract enough, excess skin, small folds or contour irregularities may remain. A dip beneath the nipple caused by over-resection is another possible issue.

For these reasons, even in well-selected candidates some people need a revision, such as additional liposuction or conversion to a periareolar technique. Read more in our guide on top surgery scars.

Recovery, risks, pros and cons

Recovery is usually similar to or slightly quicker than with other techniques: roughly 1–2 weeks before desk work and about 4–6 weeks before strenuous exercise, depending on your surgeon's advice. See our top surgery recovery guide.

Risks include hematoma (which can be harder to drain through small incisions), seroma, infection, sensation changes, dents or contour irregularities, residual skin, asymmetry and the need for revision.

ProsCons
Minimal, barely visible scarsSuitable for very few people
Nipple sensation preserved more oftenNo skin removal or areola reduction
No grafts to care forPossible residual skin and irregularities
Often a quick recoveryRevision likelihood not negligible

Frequently asked questions

You need a very small chest, very elastic skin, no excess skin and nipples that are already well positioned. Only a surgeon can confirm this after an examination or review of standardised photos.

Yes, but minimal ones: a small scar along the lower half of the areola, which is often hard to see once mature.

Usually not. If you want a smaller areola or skin removal, the surgeon will typically suggest the periareolar technique.

Even among well-selected candidates, some people need a revision, for example for residual skin or irregularities. Your surgeon can give a more specific estimate after assessment.

Long-term, very tight binding may reduce skin elasticity, which is crucial for keyhole. It is worth discussing with your surgeon during the consultation.
