Top surgery in Germany is often performed at centres with long experience in gender-affirming surgery. For people insured in Germany, statutory health insurance may cover it; visitors from abroad usually pay privately.
Is Germany a good place for top surgery? The country has university hospitals and private centres with established experience in chest masculinization. People with German statutory health insurance can apply for coverage; patients from abroad usually pay themselves, as an indication around €5,000–9,000, depending on technique and clinic.
Top surgery in Germany: a structured system
Gender-affirming surgery has been performed in Germany for many years at university hospitals, general hospitals and private plastic surgery practices. A national guideline (AWMF S3 guideline 138-001) covers diagnosis, counselling and treatment for trans people. Since November 2024, the Self-Determination Act (SBGG) also allows changes to legal gender entries without any surgical requirement.
As in every country, quality depends on the individual surgeon and facility. Before deciding, ask how often the team performs chest masculinization, which techniques it offers and how follow-up is organised.
Statutory health insurance (GKV) coverage for residents
For people insured with a German statutory health insurer (GKV), mastectomy can be covered. The usual route is an application to the insurer, supported by medical and psychotherapeutic reports and a surgeon's statement. The insurer may ask the Medizinischer Dienst (medical review service) to assess it using its own assessment guideline, which in many cases expects, among other things, documented psychotherapeutic support. Requirements and timelines can change, so always check with the insurer.

This insurance route to top surgery in Germany applies to people living and insured there. For a broader comparison of public options in Italy, the UK and Germany, see our guide to public healthcare routes for top surgery.
How much does top surgery cost in Germany if you self-pay?
Self-paying patients should expect, as an indication only, around €5,000–9,000 for chest masculinization, sometimes more at facilities with high hospital costs. The price depends on technique, surgeon, anaesthesia, nights in hospital and what is included (consultations, compression vest, follow-up). Travel and accommodation are extra.

Living elsewhere in the EU: cross-border care and the S2 form
EU rules offer two main routes for treatment in another member state with support from your home system:
- Cross-border healthcare directive: you generally pay first and claim reimbursement afterwards, usually up to what the same treatment would cost at home. Prior authorisation is often required for treatment involving a hospital stay.
- S2 form (social security regulations): with authorisation from your home insurer you are treated in the other country's public system on the same terms as its insured people. It usually applies to treatment covered at home that cannot be provided within a medically justifiable time.
In practice these routes are complex and approval is not guaranteed, since it also depends on the criteria of your home system. Ask your insurer and national contact point before travelling. If you live in Italy, our page on top surgery in Italy explains the public route there.
Techniques, language and length of stay
German centres offer the established techniques: double incision with free nipple grafts, periareolar and keyhole top surgery for small chests with good skin elasticity. The technique is chosen with your surgeon based on chest size, skin elasticity, nipple position and your goals; see all top surgery techniques for an overview. WPATH SOC8 does not require hormone therapy for masculinizing chest surgery, but the risks (haematoma, seroma, infection, nipple graft loss, changes in sensation, scars) should be discussed openly, as should stopping smoking.
German is the main language; in larger cities many clinics communicate in English. Plan to stay at least 7–10 days, until the first check-up and removal of any drains. The milestones of top surgery recovery are the same everywhere.
Germany or another destination?
Top surgery in Germany offers experienced centres and a clear legal framework, but for people not insured there the costs are similar to or higher than elsewhere in Western Europe, and well-known centres can have long waiting lists. Alternatives include top surgery in Turkey, top surgery in Spain or destinations further away such as Thailand. Compare them in our overview of top surgery destinations, and find preparation and scar care in the complete top surgery guide.
