Top surgery in Spain is a nearby option for many Europeans: private clinics in Barcelona and Madrid, short flights and a public system organised by region. Here is what to know about costs, routes and planning.
Can you have top surgery in Spain? Yes. Private plastic surgeons in Barcelona, Madrid and other cities offer chest masculinization, with indicative prices of roughly €6,000–9,000. Spain's public system also provides routes through regional trans health units, but these are generally designed for people insured in Spain and waiting times can be long.
Top surgery in Spain: the overview
Spain is often seen as one of the more trans-inclusive countries in Europe. Law 4/2023 on the real and effective equality of trans people introduced, among other things, legal gender recognition without medical requirements and includes measures related to healthcare. How such rules apply in practice can vary, so always check up-to-date official sources for legal questions.
For visitors from abroad, top surgery in Spain almost always means the private sector. Barcelona and Madrid in particular have plastic surgery clinics used to international patients. As anywhere, quality depends on the individual surgeon and facility, not on the country itself.
Public healthcare: regional units and differences between regions
Healthcare in Spain is largely run by the autonomous communities. Several regions have set up units or services for trans health, which may include chest surgery. Access routes, criteria, waiting times and the services offered differ from region to region, and waiting lists can be long.
These public routes are mainly for residents insured in the Spanish system. If you live in another EU country and hope to be reimbursed for surgery in Spain, check with your own health insurer and national contact point for cross-border healthcare first: EU rules exist, but prior authorisation is often required and reimbursement is usually tied to what your home system would cover. How much does top surgery cost in Spain?
As an indication only, chest masculinization at Spanish private clinics is often around €6,000–9,000. The price depends on technique, surgeon experience, facility, number of nights in hospital and what is included: consultations, pre-op tests, compression vest and follow-up visits. Flights, accommodation and any extended stay are usually extra.

To compare offers fairly, always ask for a detailed written quote. Our guide to top surgery cost in Europe and Turkey explains what to check.
Techniques, consultation and requirements
The techniques are the same as elsewhere in Europe: double incision top surgery with free nipple grafts, periareolar, keyhole and nipple-sparing variations. The technique is chosen with your surgeon based on chest size, skin elasticity, nipple position and your goals. You can compare all top surgery techniques in our procedures section.
Many clinics start with an online consultation based on photos, followed by an in-person visit before surgery. Under WPATH SOC8, the key criteria are capacity for informed consent, marked and sustained gender incongruence, being of legal age and other significant health conditions reasonably controlled; hormone therapy is not a prerequisite for masculinizing chest surgery. Some clinics may ask for a letter from a health professional. Every operation carries risks (haematoma, seroma, infection, partial nipple graft loss, changes in sensation, contour irregularities, scars that may need revision), so stopping smoking and keeping realistic expectations matter.
Travel, language and length of stay
Proximity is a major plus: from much of Europe, flights to Barcelona or Madrid take around two to three hours, and EU citizens only need a valid ID card. Bring your European Health Insurance Card (EHIC), but remember it covers unplanned, necessary care, not the planned operation itself.

Language can be a hurdle. Spanish is spoken everywhere and Catalan is also used in Catalonia. Many private clinics work with international patients in English, but do confirm who will be your contact if questions or complications arise.
A stay of at least 7–10 days is usually recommended, until your surgeon has checked the wounds and removed any drains. The advice on top surgery recovery applies abroad too: no lifting with your arms, compression vest and regular check-ups.
Spain or another destination?
Spain offers proximity and a generally welcoming environment, but private prices are similar to other Western European countries. People looking for lower costs often consider top surgery in Turkey with packages including hotel and transfers, while others look further afield, for example at top surgery in Thailand. Our overview of top surgery destinations in Europe and worldwide compares them, and the complete top surgery guide covers preparation, recovery and scars.
