Clear answers on techniques, requirements, recovery and costs

Top surgery FAQ

Our top surgery FAQ answers the questions we hear most often about FTM chest masculinization and breast augmentation – from choosing a technique to aftercare from home.

This top surgery FAQ covers the most common questions about techniques, eligibility, pain, recovery, scars and cost. In short: the technique is chosen with your surgeon based on your anatomy, hormones are not required for chest masculinization, initial recovery takes a few weeks, and the price depends on the country, surgeon and included services.

Top surgery FAQ: how to use these answers

This page brings together the questions we are asked most often by people considering top surgery: chest masculinization for trans men and transmasculine people, options for non-binary people and breast augmentation for trans women.

The answers are deliberately short and general. Every situation is different: the technique, recovery time and result depend on your anatomy, your health and your goals, and should always be discussed with your surgeon. For more detail, see top surgery: techniques and procedures and the articles in our guide. If you cannot find your answer, request a free consultation and we will get back to you personally.

Frequently asked questions

The main ones are double incision, periareolar, keyhole and buttonhole/inverted T; there are also non-binary approaches and breast augmentation for trans women. The technique is chosen with the surgeon based on chest size, skin elasticity, nipple position and goals.

Adults with marked and sustained gender incongruence who can give informed consent and whose other health conditions are reasonably well controlled, as set out in the WPATH SOC8 standards. Some surgeons may ask for a letter from a mental health or health professional. Details in our guide to requirements.

Not for chest masculinization: hormone therapy is not a prerequisite. For breast augmentation in trans women a period of feminizing hormone therapy, often around 12 months, is generally suggested so that breast growth can develop – this is a recommendation, not an absolute rule.

Yes. Our services and any surgery arranged through TopSurgery.it are exclusively for people aged 18 or over.

Surgery is performed under general anaesthesia, so you feel no pain during the operation. In the following days tightness, soreness and discomfort are common and are usually well managed with the prescribed pain relief. How it feels varies from person to person.

As a rough guide, many people return to desk work after about 2 weeks. Heavy lifting, raising the arms high and intense exercise are generally avoided for 4–6 weeks, and a compression vest is often worn for several weeks. Scars mature over 12–18 months. Read our recovery guide.

It depends on the surgeon and technique. Many use drains for a few days to reduce the risk of seroma and hematoma; others do not. If used, they are usually removed during your stay in Istanbul before you fly home.

It depends on the technique: double incision leaves two horizontal scars below the pecs, while periareolar and keyhole leave scars around or below the areola. Scars start out red and fade over time; how they develop also depends on individual tendency and care. See top surgery scars.

Changes in sensation are common, especially with free nipple grafts. In many cases sensation partly returns over months or years, but this cannot be guaranteed. Techniques that keep the nipple on a pedicle, such as keyhole, periareolar or buttonhole, may preserve more sensation where anatomy allows.

Most people stay in Istanbul for around 7–10 days, until the post-operative check and any drain removal. The surgeon confirms when it is safe to fly; on the flight it helps to move around, drink enough and follow the advice on preventing blood clots. As a rough guide, chest masculinization packages in Turkey often range from 3,500 to 6,500 EUR depending on technique, surgeon and services. The written quote states what is included (usually surgery, anaesthesia, hospital stay, hotel, transfers, interpreter, compression vest and check-ups) and the payment terms. See top surgery cost; to compare prices with other countries, explore our top surgery destinations or read about top surgery in the USA.

Once you are home we coordinate remote check-ups with the surgical team by photo and video call; for anything urgent you should also contact a local doctor or emergency department. If excess tissue, asymmetry or scars remain after full healing, a revision can be considered.
